The Insurance Documents You Should Keep:

  1. The member handbook provides all you need to know about your plan coverage and benefits. It provides details on how to contact your insurance, how to file an appeal, and what services are not covered under your plan.
  2. The summary of benefits breaks down how much you and your insurance will have to pay for services.
  3. The certificate of coverage is a legal document that proves you have medical insurance coverage and includes the date you became active under your plan.

Don’t worry if you don’t have these documents. You can obtain them by contacting the phone number on the back of your insurance card and requesting that the documents be mailed to you. Also, you can log into your online account to view all of the documents.
