Community Health Workers

Our frontline community health workers are trusted members of the community they serve. Each day they are building individual and community capacity by increasing health knowledge and self-sufficiency through activities such as outreach, community education, informal counseling, social support, and advocacy. Community Health Workers promote health by:

  • Serving as a liaison between communities and health care agencies
  • Providing guidance and social assistance to community residents
  • Enhancing community residents’ ability to communicate effectively with health care providers
  • Providing culturally and linguistically appropriate education
  • Advocating for individual and community health
  • Providing referral and follow-up services or otherwise coordinating care
