The mission of Care and Prepare is to stimulate end-of-life conversations through education. Care and Prepare helps individuals create medical directives, choose their “champion” or medical power of attorney, and document other important medical decisions. Care and Prepare’s leadership includes social workers, attorneys, medical providers, religious leaders, and laypersons who advocate for education about end-of-life planning.
